Every student athlete must have a valid sports physical to participate in any practices or regular season competitions.  Please click on the attachment to get an OHSAA Sports Physical Form to take to your doctor to complete.  This completed form, signed and dated by your doctor, must be turned in to the Athletic Office at the High School  for 9th -12th grade students and the Main Office at the Middle School for 7th-8th grade students.
